Description
We are looking for an Occupational Therapy Assistant Practitioner to provide a crucial contribution to the provision of high quality, life changing care to service users under the Suffolk Forensic Community Mental Health Team (FCMHT).
Along with FCMHT and inpatient colleagues, you will provide in-reach support for service users preparing for discharge, and enhanced support to facilitate reintegration into the community after discharge.
You will be involved in liaising with the inpatient team to ensure continuity of Occupational Therapy interventions during transition to the community setting.
Within this role you will be working under the supervision of a registered Occupational Therapist.You will work with service users at all stages of the Occupational Therapy Process and would be expected to work closely with the wider MDT as part of the assessment and treatment process.
