Description
Venepuncture for patients in clinic and home settingsOrganising equipment and managing diary for home visits
Recording details of blood tests in electronic patient record
Liaising with clinical and non-clinical colleagues to ensure continuity of care
Travelling independently around the county of Gloucestershire
Taking blood samples from patients under the care of Long Term Condition services in order that treatment can be adapted and progressed in a timely manner.
Travelling to clinic locations and patient homes to take blood sample, adapting diary as needed in light of non-access or late cancellation
Writing accurate and contemporaneous notes on electronic patient record
Liaising with Long Term Conditions teams to update after each visit to ensure seamless continuation of care
