Description

The Trauma Rehabilitation Coordinator will work as an integral member of a highly skilled multidisciplinary team to co-ordinate the trauma rehabilitation pathway.

The post holder will be the single point of contact in respect of the rehabilitation pathway for the trauma patient.

They will work across the health board and with the Major Trauma Centre [MTC] to co-ordinate the care pathway of all trauma patients who require specialist rehabilitation.

This would include:
- co-ordinating the patient's care while in the acute setting
- assisting in facilitating the transfer to appropriate interim facilities
- discharge home
