Qualified Social Worker - Cheshire West and Chester Council - Hospital Discharge Team - Negotiable Rate
We are looking for social workers to be part of our growing Hospital Discharge Team.

These roles are primarily to assess patients at the Countess of Chester Hospital but workers could be asked to flex as required to support discharges from other hospitals.

It is a really positive team and we work closely with out health colleagues.

Ability to undertake Care Act Assessments, MCA and Best interest decisions. Support family meetings and work with IMCA and IDVA as required. Safeguarding experience is also required. Assessments are now taking place on the Wards. To support a duty role forthe team.
