Description
The End of Life coordinator will work as part of the EPC team

Supporting the assessment and management of patients and their families needs at the end of life


The Post holder will ensure that staff across varied settings have access to the right information and support in order to deliver a seamless model of care.


Working with the End of Life Facilitator to provide advice and care planning of Patients with Palliative End of life needs.

Liaison with the Specialist Palliative care if there are complex issues

Liaising with District Nurses and Rapid response team

Management of the HCA workforce that provide a sitting service for EOL Patients

Advance care Planning

Communication and coordination with all relevant team members in both NELFT and Barts Health

COVID-19 Vaccination

Probationary Period

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust.

All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment.

As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes.

The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - London


This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum pro rata).

EPIC is a small team that works closely with the all the providers to provide excellent End of life care

We offer support to families and Patients at the end stages of Life

We complete a training programme to help with development within the role.
