An exciting opportunity has arisen to work within a dynamic, innovative and friendly team within Wirral Community NHS Foundation Trust's: Short Term Assessment Reablement Team (STAR).

The Team is going through a very exciting period of transformation within the Trust and an excellent opportunity has arisen for a number of permanent 30-hour posts for Assessment and Reablement Officers to join our reablement services.


The Short Term Assessment Reablement Service (STAR) is pivotal within the role of early intervention, admission prevention and improved wellbeing; this is integral to the requirements within our services in relation to supporting adults over the age of 18, carers, families, people and people with disabilities, long term health conditions and frailty.


The role will involve assessing and working with patients with a wide variety of complex care requirement, the role involves working as part of an integrated multi-disciplinary service, to ensure the best possible outcomes are achieved for Patients, Carers and their Families.


The post holder will be expected to undertake duties in line with service delivery and actively participate in continuous development activities.

There will be the opportunity for regular supervision and to attend in-service training for personal development.


The service operates with extended hours 7 days per week, covering the hours of 8-8 and weekend cover on a Rota basis however these may change at times depending on the demands of the service.


You need 3 years proven experience of working in a Health or Social care environment role, have an excellent understanding of current social care legislative practices.

You will be required to work in compliance of the requirements of The Care Act 2014.

To be able to risk manage, have excellent communication skills and the ability to prioritise workloads, this often requires working at a fast pace to achieve outcomes.

You will also need to demonstrate good time management and have a flexible approach to rotas across the various teams within STAR.
