Outreach Officer - ASC Contact Centre 37 Hours per week (BCP Grade G)
Do you have the enthusiasm, drive, and commitment to help transform our Adult Social Care Contact Centre?


We have a full-time permanent Outreach Officer post that will develop prevention and early intervention initiatives to reduce, delay or remove the need for support passing to long-term Adult Social Care teams.

The ASC Contact Centre currently consists of fourteen Contact Officers and ten Outreach Officers, alongside management support.

The aim of the ASC Contact Centre is to respond to Adult Social Care queries and referrals concerning residents in our area.


These initiatives will contribute towards our Vision:

  • The Adult Social Care Contact Centre is here for you when you or someone you know needs advice, information or support about how to manage day-to-day with a disability, illness or long-term condition. _
  • We listen carefully to understand what matters most to you, help you navigate life events and find answers including; connecting you to your community, staying in touch with family and friends and solutions so you can live as independently as possible._


The Outreach Officers play a pivotal role in helping to design and evaluate innovative ways to respond to customer demand and, through a process of testing and learning, contribute to a growing evidence base of what works best.


As an Outreach Officer, you will be confident in accessing and making use of web-based, community resources and understanding the benefit of voluntary and local services to a person's wellbeing.

You will be expected to receive referrals from Contact Officers within the team and complete virtual/telephone or face to face contacts with adults and carers, to discover their desired outcomes and needs, co-produce plans and review interventions.


This represents an ideal opportunity for those with an interest in promoting early intervention approaches and improving outcomes for residents, often in a fast-paced environment where rapid decisions are regularly taken, and teamwork is of utmost importance.
