This post is an exciting opportunity to join the Wakefield 0-19 Team as an Psychological Wellbeing Practitioner, The post holder will assess and deliver outcome focused, evidence-based interventions to children and young people experiencing mild to moderate mental health difficulties.


Working in partnership they will support children and young people experiencing mild to moderate mental health difficulties and their families in the self-management of presenting difficulties.

Providing a range of information and support for evidence based psychological treatments. This may include guided self-help. This work may be face-to-face, by telephone or via other media

  • Assess and support people with a common mental health problem in the selfmanagement of their recovery.
  • Work in partnership with children, young people and families in the development of plans for the intervention and agreed outcomes.
  • Support and empower children, young people and families to make informed choices about the intervention.
  • Undertakes accurate assessment of risk to self and others.
  • Undertake patientcentred interviews which identify areas where the person wishes to see change and/or recovery and make an accurate assessment of risk to self and others.
  • Make decisions on suitability of new referrals, adhering to referral protocols, organising assessment, signposting to appropriate services and facilitating referral to specialist services where required.
