TristoneNash are pleased to be supporting a reputable Housing Association to appoint a Floating Support Worker for this full-time, 3 month contract with the possibility of extension.

You will deliver a comprehensive needs led, Support Service which enables people to, wherever possible, remain within their home and lead an independent, full and varied life through actively providing advice, assistance as part of the individual personal plan to ensure that people access timely support and the right services from the relevant health, social, housing and voluntary sector agencies.

You will actively engage with and involve all residents of Torfaen who are 16+ with a low to medium level housing related support need i.e.

Social housing tenants, private renters, homeowners and homelessness.

A welldeveloped knowledge of housing related support services
Knowledge of different methods of assessing or supporting need
Experience of delivering support to people including; support planning, risk assessing, needs assessing and delivering high quality floating support
Experience of working within a supporting people framework including working with goals and outcomes.
Experience of effectively supporting people to build resilience and promote their independence.

Salary:
£13.00-£16.00 per hour
