Your role will be to support the team with service delivery supporting with clinical interventions and providing care to vulnerable individuals.
The District Nurse team promotes wellbeing and independence to all our patients in an understandable and approachable way.You must be able to display effective communication skills with patients, family, carers as well as the wider Multi professional workforce.
The Health Care Support Worker for the District Nursing Service will assist the team in the delivery of high quality nursing care to patients in the community setting.

To undertake delegated duties to patients in their own home, residential care setting or clinic setting, this can include routine wound care, venepuncture, assisting in the delivery of palliative care, administration of insulin and eye drops following training and assessment of competencies.
Communicates with the District Nursing Team members, other health care professionals and voluntary agencies discussing patient care as directed by the qualified nurse.
Undertakes clerical work within the team as delegated by the Team Leader such as stock ordering, filing and retrieval of patient's notes.
To participate in health promotion programmes with patients and carers, encouraging self-awareness and self-management.
To assist in the support and education of family/carers to prevent ill health.
Completion of the Care Certificate will be required.
Communications
Maintain confidentiality of information.
Communicate effectively with staff at all levels including team members, patients, relative and other visitors to the Trust, acting at all times within the Trust Values and Behaviours.
Able to identify and develop methods to manage barriers to effective communication.
Demonstrate excellent verbal and non-verbal communication skills to communicate sensitive information effectively with patients, families, colleagues and other agencies.
Communicate appropriate information to and from other departments/service as required.
Maintain accurate and comprehensive patient records in line with organisation standards of practice and data protection legislation.
Demonstrate empathy with patients, their families and carers, ensuring their views, choices and experiences influence thinking and individualised care provision.
Contribute to the budget management by ensuring that the most effective use of resources is maintained and reporting any anomalies to the Team Leader.
Promote a safe environment for the treatment of patients and protection of staff by ensuring the effective use of equipment and materials in compliance with Health and Safety Policies.
Ensure the regular maintenance of technical equipment including calibrating where required and checking for faults.
Assist in the organisation of efficient stock control, maintaining adequate levels of stock and promoting the most cost effective usage of these resources
