Description
To undertake a service leadership role in planning, co-ordinating, delivering, developing and evaluating the services.- To ensure effective, efficient delivery of services.
- To provide a Clinical Lead role providing direction and professional advice on all matters relating to Adult In-Patient Therapy Services within Bolton FT Foundation Trust.
- To performance manage the service and take appropriate effective action when necessary, liaising with the Service Manager and producing regular performance reports, ensuring early escalation of any issues.
- To be responsible for Clinical Governance meetings and aspects of the service including managing departmental research and audit activities timely and effectively.
- To maintain and develop communication networks with key players within partner agencies including Bolton FT NHS Foundation Trust, Social Services, GP's, community and voluntary services/agencies to develop policy and procedure for multiprofessional practice for people.

DUTIES AND RESPONSIBILITIES
Clinical
- To lead a team of Physiotherapists, Occupational Therapists, Therapy Instructors, Therapy assistants and Administrative Professionals to provide a Service for people who require therapy in hospital
- To provide a highly specialist therapeutic service to people with diverse presentations and complex physical and psychological needs within hospital and community settings guided by RCOT, CSP, RCN, GMC policies, standards and regulations.
- To manage a caseload, and work as an autonomous practitioner using evidence based/people centred principals to assess, plan, implement and evaluate interventions.
- To provide expert clinical guidance to the Multidisciplinary Team (MDT) and other professionals within Bolton FT NHS Foundation Trust and beyond with regards to complex therapeutic intervention, discharge planning and case management. To liaise with after, inform and train staff, whilst working in partnership with patients, relatives/carers, health and social services staff and members of voluntary organisations and the community to ensure delivery of a high quality service.
- Be professionally and legally accountable for all aspects of own work, including the management of patients.
- Monitor and evaluate the appropriateness of team referrals, ensuring appropriate care, discharge and communication is provided.
- As an autonomous practitioner to independently prioritise, organise and manage own specialist caseload. To demonstrate skills in dealing with complex issues and generating appropriate strategies for case management across a wide range of fields.
- Undertake a comprehensive, highly specialist assessment of patients, including those with diverse or highly complex presentations; use advance clinical reasoning and problem solving skills. Collect evidence from both formal and informal assessment using highly specialist knowledge and expertise.
- Represent the service and/or individual patients at multidisciplinary team meetings, to ensure the delivery of a coordinated multidisciplinary service. This will include discussion of patient care, patient progress and involvement in discharge planning.
- Provide professional and clinical advice to other health care professionals and organisations including Consultant's, GP's and nurse practitioners.
