We are looking for a highly motivated Mental Health Nurse. Are you looking for a new challenge? Are you looking to broaden your existing skills and knowledge around continuing healthcare, individual patient activity, personal health budgets and funded nursing care?

We have a new Band 5 CHC Practitioner, working across various sites around the Derbyshire region. This is an opportunity to join an established review team.

Duties will include reviewing patients with complex care needs who are eligible for NHS Continuing Healthcare funding or NHS-funded nursing care.


The posts are agile with a requirement to attend the office as and when required and there will also be a requirement to complete some assessments face to face.

A vehicle for the purpose of business travel is essential for this role.

Experience of undertaking assessments, and working with patients, families and carers is essential.

The Personalised Healthcare Team are responsible for the assessment and review of people who reside in nursing care homes and the clinical commissioning of packages of care for people with continuing healthcare needs who reside in their own homes.
