Building on the existing Liaison and Diversion model to encompass the criminal justice pathway and beyond the prison gate. We aim to support individuals with identified needs on release from prison known as "Reconnect". This project will operate under the wider auspices of the Hampshire and IOW Liaison & Diversion Service.

Do you believe that a person's health needs, and past experiences can be linked to their criminal offending? By identifying and reducing health needs can vulnerable people be supported to make positive changes and reduce the risk of reoffending?


We're looking for a Mental Health nurse, Learning Disabilities nurse, Occupational Therapist, or Social Worker to join this service to and be able to work flexibly across Hampshire to support the Reconnect Lead Practitioner manage a small team and the day to day running of the Reconnect project.


You will ensure the team respond professionally to all requests for information and assistance, whether in person or by phone, using discretion as to appropriateness of requests and being aware of confidentiality and information sharing guidelines at all times.

This information may be complex, sensitive, or contentious requiring negotiation, persuasive and discernment skills.


Recognised with several awards, including Hampshire Constabulary and Thames Valley Police Commendations, Berkshire Healthcare 'Team of the Year' and runner-up for the Howard League for Penal Reform 'Liaison and Diversion Service of the Year'.
