An exciting opportunity has arisen for a a new and innovative role as a pathway navigator to work alongside our Hepatology team.

This role will be across county when required.


The National Cancer Team has allocated funding to cancer alliances to support providers to establish systems to processes to invite those legible for liver surveillance where these do not exist.


The aim is to greatly improve liver surveillance, reduce health inequalities,identify more people at high risk of liver cancer and to diagnose more liver cancers at an earlier stage.


We are seeking to recruit a highly motivated, confident and dynamic individual who will be pivotal in establishing a more robust system so that through project management and data collection there will be improvement in our diagnostic rates and help reduce patient cancellation, non attendance for tests and clinic appointments therefore influencing progress through a a much more rapid diagnostic pathway.


Working under the guidance of registered health care professional, this role is to ensure that our patients with liver disease whom are at great risk of developing liver cancer are kept under hepatocellular surveillance and that surveillance investigations are carried out in a timely manner.


The pathway Navigator will be expected to co ordinate such investigations and track the progress of patient care through the diagnostic phase, pursuing results and ensuring all results are managed appropriately to avoid potential delay.

Maintain and update relevant databases as required

To actively engage in service improvement actions across the pathway which maximize patient experience and outcomes.
