What is community mental health transformation


As part of the national mental health transformation plans, the Primary Care Mental Health Hub is a new, place-based, multidisciplinary community mental health service for adults and older adults in Doncaster.

RDaSH NHS Foundation Trust, Primary Care Doncaster, Local Authority and the Voluntary Community and Social Enterprise Sector (VCSE) have been working together over the past two years to deliver this partnership approach to primary care mental health.


There is a central base for the new multidisciplinary team to come together but intervention will be delivered in local community bases.

Our Primary Care Mental Health Hub comprises new roles such as CAPs (Clinical Associate Psychologists), MHWBs (Mental Health Wellbeing Practitioners), Primary Care Mental Health Practitioners, Community Connectors as well as the more familiar roles of the Clinical Psychologist, CBT Therapist, Occupational Therapists, Nurses, Social workers, and Peer support workers.


The aim of the team is to provide personalised, trauma-informed care at primary care level to improve access to evidence-based psychological interventions, valued activities in people's local communities, and medicine management.

You will be joining this developing workforce at an important time and there will be opportunity to be part of shaping service delivery working alongside statutory organisations and VCSE partners.
