Description

The post holder would be expected to cover the home exercise programme as required to meet the needs of the service.

This would involve facilitating exercise with patients in their own homes after they have been assessed by the lead Physiotherapist.

To deliver group based cardiac rehabilitation sessions in line with current BACPR guidelines at four centres across the city.

To provide individualised and evidence based cardiac rehabilitation programmes in a patients own home or at the four centres on a one-to-one basis as deemed necessary.

Provide advice and support to patients with a wide range of cardiovascular conditions. To assist and advise other members of the cardiac rehabilitation Team around their exercise prescriptions. To manage a caseload under the direction of the cardiac rehabilitation nurse or physiotherapist.

Support individuals to make informed choices about their health and wellbeing and supporting them in initiating and sustaining appropriate exercise behaviour changes to improve their health.

Deliver functional assessments of all patients referred to the cardiac rehabilitation service. Deliver educational sessions during a cardiac rehabilitation course on physical activity and the benefits of exercise. To carry out an assessment the cardiac rehabilitation centres. To provide and deliver individualised treatment programmes.

To accurately record rehabilitation progress as per Trust documentation / database including outcome measures.
