Are you looking to work with Adults? We have an exciting opportunity for you to work as Social Worker in Continuing Health Care Services.

The pay rate would be up to £40 per hour depending on your location and this may be negotiable.


Your key responsibilities will include:


  • To assess people, already receiving services from RBC's Adult Social Care, that have been identified as potentially eligible of health funding, by undertaking a Care Act review alongside completion of a CHC eligibility checklist.
  • To undertake mostly face to face reviews, a small proportion of these will be out of borough. Reviews will be recorded as a workflow on our data management system, Mosaic.
  • To identify if the current care and support plan is appropriate for someone's needs and if there are changes it may lead to a reassessment.
Where someone has high health needs, as well as Social Care needs, a CHC checklist will be completed.


Reed are now offering a joining bonus of up to £250 for Qualified Social Workers. Terms apply.

  • Registered with Social Work England
  • Good knowledge of Continuing Healthcare guidance
  • Good knowledge of the Mental Capacity Act, Care Act and Safeguarding principles.
  • Post qualifying experience of minimum 3 plus years in working with Adults with learning disability and similar services.
