An exciting opportunity has arisen for a Primary Care Link Worker to join the team at Knowsley.

Responsibilities:

As Primary Care Link Worker you will work out in the community and will work closely with a number of partnership agencies to develop and sustain effective comprehensive treatment pathways and broaden the service users recovery plan accordingly.
