as a Social Care Practitioner.

As a Social Care Practitioner, you will work under the guidance of the Team Leader to provide high-quality care and support to adults with a range of social care needs


Your role will involve:

  • Conducting assessments and reviews to identify individual needs and develop appropriate care plans
  • Offering advice, guidance, and practical assistance to service users and their families
  • Collaborating with external agencies and professionals to ensure coordinated and holistic support
  • Monitoring and evaluating the effectiveness of interventions, making adjustments as necessary
  • Safeguarding the welfare and promoting the independence of vulnerable adults

To excel in this role, you should have:

  • A relevant professional qualification in social care
  • Experience in assessing and delivering personcentered care
  • Sound knowledge of relevant legislation and best practice guidelines
  • Strong communication and interpersonal skills
  • The ability to work effectively within a multidisciplinary team

This role requires the incumbent to have a car available at work to undertake his/her duties as they will be expected to undertake at least 1500 business miles per year.

Essential Car User status attracts a lump sum allowance paid on a monthly basis as well as a free car parking space if he/she works at one of the Council's corporate buildings.


This role has been identified as public facing in accordance with Part 7 of the Immigration Act 2016; the requirement to fulfil all spoken aspects of the role with confidence in English applies.
