We have an exciting opportunity for you to join our Neonatal Outreach Team. You will need to be a register nurse and hold a qualified in speciality qualification for neonatal care. Your role will predominately be based in the community, supporting and caring for discharged neonates and their families. You will also be accepted to support and liaise with colleagues on the neonatal unit. You will need to have excellent communication skills and be able to work autonomously.

You will need to have experience of teaching / mentorship to enable you to teach parents and assess babies.

There is an expectation that you will also share your knowledge and experience with the whole of the neonatal team.

The ability to speak Welsh is desirable for this post. English and /or welsh speakers are equally welcome to apply.

However, overview of main duties is

  • Manage autonomously an individual caseload of neonates and their families.
  • Assess, Plan, Implement and Evaluate the care of the neonate.
  • Work in collaboration with other members of the outreach team, neonatal team, multi-disciplinary team and other agencies.
  • Follow safeguarding guidelines.
  • Provide family integrated care, promote health promotion directives and support the well being of the family.
  • Deliver best evidence based practice.
  • Act as a link nurse for the outreach team, in relation to the teams additional roles / responsibilities e.g. development clinics.
  • Participate in audits, research, development of guidelines, as required.
  • Provide and / or support with teaching sessions for staff and students.

As a Neonatal Outreach sister your main responsibilities will be detailed under the following categories

Managerial

  • Manage autonomously an individual caseload, prioritising workload, delegate as required and cover responsibilities in the absence of the Band 7 neonatal Outreach Senior Sister. Undertake risk assessments in the patients home and follow lone worker guidelines.
Clinical

  • Provide individualised family integrated care, whilst assessing, planning, implementing and evaluating the care provide. Perform clinical investigations, following guidelines and best evidence based practice. Maintain accurate records (paper and / or IT), ensure confidentiality. Arrange and manage care for complex neonates in the community, liaising with the multi
- disciplinary team and other agencies when required. Support the admission to discharge process and act as an advocate for families. Carry equipment required for home visits, ensuring manual handling guidelines are followed at all times.

Organisational

  • Follow trust policies /guidelines at all times and participate in development / review of outreach guidelines. Support team working, innovation and practice development. Promote new ways of working.
Research and Development

  • Promote and participate in evidence based care. To contribute / perform research to improve the outcomes and care for neonates and families.
Audit and Quality

  • To contribute to setting standards and monitoring measurable outcomes. To participate in the audit process for monitoring and reviewing quality. To manage complaints in accordance with trust guidelines.
Legal and professional

  • To adhere to Trust and departmental policies and guidelines. To provide statements where requested. Keep up to date with mandatory training and NMC requirements. To sustain and improve own professional development.
