Are you looking for a new and exciting role that can really make a difference to people's lives? If you are passionate about increasing engagement whilst also understanding barriers for groups including carers, veterans, ethnic minorities, people living with disability, mental illness and other complex needs, this role could be for you.

We are looking for a self-starter able to lead and motivate others to provide best effective outcomes. This role will be hybrid and you will need to enjoy working in a variety of different environments and being part of wider support teams.

Purpose of role:

Some groups of people have more barriers to overcome in order to increase engagement with statutory and community services.

We are looking for a committed individual to understand the barriers, how they impact and their potential links to the wider determinants of health.

Successful outcomes will be achieved through collaborative working between health, social care, Voluntary, Community and Social Enterprise (VCSE) sector. Appropriate referrals to relevant services will also be an essential part of this role.


You will work closely with individuals, GP practice staff and the Social Prescribing team to ensure needs and specific requirements are met.

You will be a lead point of contact and provide accurate and relevant information to inform future development of the role.


Main responsibilities (Job Description)

  • Assisting individuals to recognise their own health and wellbeing needs
  • Support people to manage their needs through answering queries, developing support pathways and ensuring that people have good quality written or verbal information to help them make informed choices
  • Work with GP practice staff to improve the levels of engagement of people who experience barriers to accessing support
  • Work as an effective and responsible team member, supporting others and exploring mechanisms to develop new and effective ways of working across organisations
  • Create opportunities to improve support and choice through liaison with individuals, families, relevant professional and support organisations
  • Support the equality, diversity and rights of individuals, families and colleagues and act in a way that recognises the importance of peoples rights, respecting the privacy, dignity, needs and beliefs of every individual
  • Be responsible for and committed to safeguarding and promoting the welfare of individuals ensuring that they are protected from harm
  • A commitment to Continuous Professional Development and undertaking relevant training opportunities when they become available
  • Experience of working in or with Health and/or social care environments
  • An understanding of the wider determinants of health, including social, economic and environmental factors and their impact on individuals, their families and carers
  • Experience of developing new services and/or projects
  • Experience of hybrid working and/or as part of multiple teams
  • A passionate commitment to personalised care and enabling choice for all
  • Possess excellent communication skills and the ability to sustain good working relationships across many teams
  • Experience of following legal, ethical, professional and organisational policies/procedures, and codes of conduct and personal responsibilities to current legislation and adherence to policies and procedures on Safeguarding
  • Excellent holistic assessment skills and experience of providing support to adults in a planned and structured way
  • Excellent IT and record keeping skills along with the ability to complete appropriate administration for organised meetings.
  • The ability to work autonomously, to plan, prioritise work under pressure and adapt to new models of working

In addition to the contents of the role description, employees are expected to undertake any and all other reasonable and related tasks allocated by line management.

£22,741.00 per year
