I am highly focused and conscientious person with a decade and half of working experience in Health and Social Care and a Newly Qualified Social Worker. I am presently working as a Registered Branch Manager for a domiciliary care provider.

Self-motivated and driven to provide the best possible support for clients in need of full or part time assistance. Creates health plans based on the individual needs and implements high standard of care no matter how challenging the circumstances. 

Supervising interdisciplinary care by bringing together the different specialists whose help the patient may need, also responsible for monitoring and evaluating the care delivered. 

Able to adapt to the broad range of situations that can arise and comfortable working independently or as part of a team and a team leader

• Supporting with assessment/ access to care on discharge from the hospital

• Supporting vulnerable adults in applying for benefits and checking what they are eligible for
• Using the Care Acts to check their legibility
