I am a dedicated, compassionate support worker with over one year of experience supporting vulnerable service users (adults and children) with complex needs and learning disabilities, improving their health and well-being, and increasing the level of their independence to helping them reach their full potential.
In my previous workplaces, I performed various functions including delivering personalised support and encouraging fun and creativity for vulnerable individuals with learning disabilities, mental health issues, and acquired brain injuries, assisting with medications, well-being, complete and maintaining necessary daily records painting a full picture of social engagement, activities, and physical care. Also, to contribute to the patient development care by supporting them to attend community therapy sessions; Headway, Day Centre and others, carrying out a monthly support review on the patient developmental progress, and getting feedbacks and views from carers/family and friends on what is working well and what not is working well, new interests and how to integrate them into their plans or treatment for further improvement.
Having previously worked in a medical setting, I understand the ability to observe, record and report patient behavioural changes/needs to the appropriate team member. To use effective communication tools to communicate treatment programmes to patients who may have difficulties in communication. I carry out these skills effectively and efficiently by good communication and written skills, being compassionate, caring, friendly, and with the ability to maintain confidentiality amongst service users.
