Hello!

 I am an ex qualified  social worker and also have extensive experience as a carer for my own parents and for private care homes and agencies. 
I drive, have a current Enhanced DBS, educated to degree level and have recently taken 35 exams all relevant to care work. 

My hobbies are travel, gardening, cooking, documentaries, history, music and inviting friends and family to dinner. Ladies day at Ascot doesn't go amiss !

I thoroughly enjoy assisting others to be what they want to be and do. 

Experience

Sept 2001-Feb 2021:Qualified Social Worker, mainly contract but held two perm posts, at Bexley and Slough Councils. 

I worked in multidisciplinary multi sector teams, Assessing all adult client groups for Care and Support, completed Risk assessments, Human Rights, Mental Capacity, Carers. Made Dols applications, applied for funding for dom care, respite, Direct Payments, nursing, residential, Supported Living. Contributed to team meetings to share research and improve practice models.

Worked/liaised with doctors, nurses, psychology, psychiatry, dieticians, occupational therapists and physiotherapists, police, fire brigade and home office, charities, Consultant Forensic Psychiatrists and private sector care companies to attain best outcomes for clients, carers and families. Advocated for clients to enable and empower them to have a say and obtain their rights and understand their responsibilities. Taught life skills and used person centred methods, Positive Behaviour Support. Completed CHC’s and joint health DST’s under NHS Framework 2012. 

Applied to Court of Protection for Health and Welfare LPA & for Inherent Jurisdiction to High Court) Sec 44 of MCA)

Supported clients set up their own user forums and leisure/learning clubs. Helped clients get work experience, paid jobs, access college courses and volunteer work. Taught life skills, travel skills, money management, Stranger Danger and how to maintain tenancies. Provided group and one to one counselling for clients in distress or crises. Done Crisis duty and out of hours emergency.  Supervised and buddied student social workers and nurses. 

Covered for Team Manager at Bournemouth  May 2010- July 2010, managing the Adult Community and Health Team to meet team, LA, Health, local and nation targets, performance indicators and priorities. Compiled Personal Budgets and Self Directed Support with clients to maximise independence, choice an control. Worked with PoHwer and Healthwatch and IMCA’s. Prepared and submitted Mental Health Tribunal Reports. Arranged care for hospital discharges and supported clients to access benefits and housing. Wrote supervision and progress reports for team managers and heads of service. Conducted Sec 42 Adult Safeguarding Enquiries, reporting back to team managers. Contributed to formulating multidisciplinary and multi sector strategies for current and future services to further improve service quality, economy, efficiency and efficacy.

July 2020- Sept 2020: Sylvian Care Live in Health Care Assistant: Joint worked with another agency to care for an elderly bed bound gentleman with dementia: Administered and recorded all meds, shopping, cooking cleaning, laundry, skin integrity and incontinence care. Provided 24/7 care including moving and handling equipment eg hoists, sliding sheets, lift belts, electric shower seats & Return. Digitally recorded updating to care plan and shift updates. Provided stimulation talking books, reminiscing, music, trips out, supporting social networks).

Sept 2018: had to leave West berks Council Adult Community Team to undergo cancer surgery. Now recovered. 

VOLUNTEER WORK/STUDENT Social Work PLACEMENT

Jan – June 2001: Medway Council Student placement: On Adult brain injury team. Completed case load management, out of borough placements, Care and MH Assessments, Risk Assessments, Care Plans, chaired BI meetings, arranged emergency and respite care Respite breaks carers assessments. Worked withing tight budgetary constraints to meet team, local and individual outcomes and meet needs. Used creative strategies such as fund raising for disability equipment.

Jan – March 2000 Clubhouse, Gravesend student Social Work placement:

Rehab and prevention work undertaken for those with severe and enduring mental health difficulties. Training and reskilling adults in preparation for independent living and paid work. Empowering and motivating clients to maximise their own potential, encouraging ownership of problem solving and to advocate for themselves and others eg supported set up of an advocacy forum for members and they decide who does what and how things are run. Supported carers to organise and manage their own carers support groups. 

Volunteer Work: Moving Forward, Gravesend, Kent 1998-1999. Mentor assisting young people to access housing, jobs, education and social support networks to maximise their confidence and potential and reduce risk of re offending.

Feb 1999 -May 1999 Prince George Duke of Kent Nursing and Residential Care Home

Senior Carer/Supervisor: Responsible for Staff supervision and training, drugs trolley and meds orders/delivery, all meds charts, application and compliance with regulations for care residents with moderate to serious nursing and social care need. Liaison with care and health professionals, joint assessments and annual reviews, 6 week reviews. Ensuring own and staff maintained admin records, contact sheets and care plans. Organised family days, visiting shops plus stimulating activity days. Managed compliments and complaints to satisfaction of all parties and ensured staff were acknowledged and rewarded for their outstanding efforts and good client/carer/service and family  care.
