We are looking for Bank Assistant Practitioners to join our established Integrated Community Stroke Service to deliver rehabilitation into various community settings.

Care is currently delivered 8am-5pm, 5 days a week, including Bank Holidays. Weekend working may be required in the future.

This role requires day-to-day delivery of patient care in our Integrated Community Stroke Multidisciplinary Team of AHP's, together you all make the difference


You will need to:
Demonstrate your passion and clinical ability to assess patients who present with significant physical health care needs.


Support our patients within the community to enable them to live their optimum life, being self-caring and independent with their health needs following a stroke.


Analyse and develop treatment plans, working collaboratively within a multidisciplinary framework, providing compassionate, realistic and timely support to a diverse patient group and their families.


Demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable, resourceful and approachable, evidencing high levels of personal and clinical credibility, being able to establish effective working relationships in a range of work settings.

We are looking for Bank Assistant Practitioners to join our dedicated Community Stroke Team across the South of Nottinghamshire.


Care provided outside of traditional hospital settings is quickly changing and becoming an integral part of services provided by the NHS.

The Community Health Services Division of NottsHC are at the forefront of this exciting future, working with our system partners in creating collaborative ways of working and improving services in local communities to better meet the individual needs of our patients in their own homes, residential homes and in local accessible clinics.


This post will allow the successful applicant the opportunity to work within an integrated multi-disciplinary team, providing therapy to patients referred to the Community Stroke Rehabilitation services, including early support stroke rehabilitation.

The multidisciplinary team includes Occupational Therapy, Speech and Language Therapy, Physiotherapy, Neuro-Psychology, Assistant Practitioners and Rehabilitation support workers.


As a Community Stroke service, we aim to offer a high quality therapy led Multidisciplinary Team approach providing stroke rehabilitation in a patient's home focused around achievement of agreed rehabilitation goals.

As an Assistant Practitioner in the Community Stroke team you would be expected to undertake therapy assessments and devise treatment plans with indirect support of the qualified clinicians.


We are an experienced existing provider of Community Stroke Rehabilitation and as part of our service we provide a review for patients after stroke at 6 and 12 months.

As an Assistant Practitioner you would be required to undertake these assessments as part of this role when required.

This encompasses completing a holistic review of all the service users health needs related to their stroke, together with advice and signposting for services.


The Division operates under three Units of Management that provides an extensive and diverse range of children and adult services, activities and interventions for people with high or complex health and care needs.
