SUPPORT PLANNER, CLUSTER TEAM, NOTTINGHAM, PERMANENT, 37 HPW, FRWK06 £20,127 - £22,293
Framework has a rare and exciting opportunity for a role in Social Care working Mondays to Fridays 9am-5pm. The Support Planner role will join a small and friendly team based in Nottingham City.


The Cluster Team supports people with complex needs (a combination of homelessness, substance misuse, mental ill-health, domestic abuse, and offending behaviour) in dispersed housing throughout Nottingham City.

The Cluster Team offers a supported introductory tenancy that will help our residents to successfully move on to their own independent accommodation.


Duties & Responsibilities

  • To provide support to Service Users in a safe and maintained environment, by support planning, reviewing and assessing their individual risks and needs.
  • You will have caseloads with Tenant's in properties across the city (mileage system for car drivers and public transport paid).
  • Prepare Support plans.
  • Sign post Tenant's onto appropriate services for substance misuse management & health.
  • Assist service users with basic budgeting and life skills in order to support them to move on to live independently.
  • To liaise appropriately with Key partners, stakeholders and service providers to ensure appropriate support is offered to all Service Users.
