Age Connects North East Wales is an independent local charity supporting older people in Flintshire and Wrexham.

We are looking to recruit an advisor/mentor to work on our older peoples support service in Flintshire.

The role will have 2 functions, firstly to advise older people (50+) through home visiting and secondly to mentor new staff and students within the organisation.


A background in housing and/or welfare rights would be preferable, along with experience of working in the community, ideally on a housing support grant project.

Full time hours available (35 hours per week) and a salary of £22,750.

We are an age friendly employer who values experience.

Applicant will need to have a full driving license and access to their own car.

An enhanced DBS check will be required for this position.


JOB PURPOSE

  • To provide information, short term support and advice to older people 50+ in Flintshire. The Service ensures that the older person feels empowered by their contact with the advisor and can remain independent and remain living in their own home and preferred community.

SERVICE OUTCOMES

  • Supporting the older person to feel safe and secure in their home, offer opportunities to rebuild and extend their skills, interests, and social networks.
  • Leaving the older person with increased selfconfidence so that they can manage their homes and lives easier.
  • Enabling the older person to feel valued and empowered by being involved in the process.

DUTIES AND RESPONSABILITIES

  • To undertake full assessments and accurately record identified assessed needs/issues and complete forms and Charity Log.
  • To record any actions required by the client/advisor on a support plan and on Charity Log, and plan delivery of these actions.
  • Ensure that the older persons view is central to the assessment and the subsequent support plan.
  • Research solutions to client's issues, linking in with other agencies and voluntary organisations and making appropriate referrals.
  • Encourage the older person to work with you to improve their situation therefore helping them to build confidence and experience to foster independence.
  • Identify eligibility for welfare benefits and assisting with completion of benefit claim form, to maximise client's income.
  • To ensure all benefit gains are recorded on the spreadsheet at least quarterly.
  • To mentor any student placements allocated to work within the service and organisation.
  • To assist in the induction and training of new staff on the project.
  • To attend HSG meetings when appropriate and to deputise for the manager of the service.
  • To liaise effectively with other agencies and organisations to ensure the coordination of support services.
  • To ensure close working links with service providers including primary health care, NHS, other voluntary organisations, independent sector organisations, housing and transport providers, thereby maintaining a coordinated approach.
  • To undertake presentations and attend information events to promote the service and other ACNEW projects to groups of older people, community groups, other voluntary organisations, and statutory organisations.
  • To raise awareness about the project and promote its aims and objectives.
  • To identify and record any unmet need and contribute to the identification of new services and community resources that would support older people.
  • To carry out other specific tasks as directed by the CEO and her deputy to support the daytoday operation of the project/ACNEW.

Person Specification

Qualifications:
GCSE grade c or above in English and Maths or an equivalent.

Advice and guidance or advocacy qualification is desirable.

Skills:
Excellent communication and time management skills.

Good IT skills, to include Microsoft Office, and online systems such as databases

Ability to speak Welsh is desirable.

£22,750.00 per year
