Description

We seek a Strategic Health Facilitator to join the Kirklees LD Community Health Team.

You will have a key role in service development, responding to national policy drivers ensuring these are imbedded in clinical practice; leading on strategic development, education, training programmes, developing and delivering health promotional initiatives to people with a learning disability, their families and carers.

Will work closely with CCG and primary care to improve and promote access to healthcare with a clear focus on improving uptake of annual health checks and be responsible for ensuring access to high quality uninterrupted healthcare for people with learning disabilities and additional health needs.

Support an infrastructure with the Acute Trust promoting inter-agency working for the benefit of improving access and facilitate timely discharge for individuals with a learning disability and complex needs.

You will be committed to delivering high quality care, always ensuring safe practice and a positive patient experience
