Due to an expansion in the team exciting opportunity has arisen for a Band 4 Home Enteral Feeding Practitioner to join and support our well-established community learning disability (LD) enteral nutrition nurses' team.
We are looking for a motivated, dynamic, and enthusiastic practitioner to be a new addition to our department and grow our current service.
The role will work with adult LD patients alongside dietitians, community-based nurses and our experienced Nutrition Nursing team support enteral feeding in adults.
The team liaises closely with acute units, enteral feeding companies, community hospitals, day centers and nursing homes in relation to home enteral feeding discharges, assessment for initiation of enteral feeding, direct patient care and training.
The successful post holder should therefore have experience in care of complex patients as well as excellent IT skills to ensure equipment is available for patients.
Good communication is essential as you will be expected to communicate effectively with professionals working in the community as well a very diverse patient caseload.
