A vacancy has arisen for a Social Care Practitioner to join us in a busy, supportive and dynamic fieldwork team.

The team consists of Social Workers, Occupational Therapists and Social Care Practitioners and supporting adults over 18 and their carers, plus transition cases from the age of 14.


You will be based within the High Peak Personalisation and Prevention Team and will be covering the whole of the High Peak area.

There is an expectation you will be office based for part of the week and to work from home.

Training and induction will be provided alongside meetings including regular management and peer supervision.


You will need to have experience and an understanding of assessing and reviewing service user needs in the community or residential settings.

Working with the short-term services is an essential requirement of this post and knowledge of reablement and goal setting would be beneficial.

You will be expected to work in multi-disciplinary partnership with other professionals, service users and their families and carers.


You will create person-centred and strengths-based assessments and support plans and commission services to enable individuals to meet their outcomes, promoting a home-first culture.

You will need the ability to travel and ideally have access to your own transport for which a mileage allowance is payable.
