An opportunity has arisen within the Breast Specialist Nursing Team based at Thirlestaine Breast Centre at Gloucestershire Hospitals NHS Foundation Trust.

Do you want to develop? We have a permanent, full time opportunity to work within our team as a Cancer Support Worker.


The Cancer Support Worker is an integral part of the team working under the supervision of the existing Clinical Nurse Specialists.


The main purpose of the role is to support people to navigate the complex health and social care system during and or following completion of cancer treatment.

The post holder will coordinate care by delivering emotional and practical support to patients with a diagnosis of cancer.

You will specifically lead in the delivery of a 'Personalised Care and Support Assessment' which aims to identify and address patients concerns.

w/c 11th September


You will work closely with the Breast Specialist Nursing team both under supervision and independently as patient interaction/intervention or practice dictates.

You will work as part of the wider Cancer Support Worker Team and have support from the team leaders.


The post holder will demonstrate excellent organisational skills, must be flexible in approach, able to exercise initiative and demonstrate a consistently high standard of professionalism, being aware of the need for confidentiality and integrity.

The post holder will have excellent communication skills.


Knowledge and skills to include:


  • Relevant Health and Social Care experience at AFC Band 3 or equivalent.
  • Efficient in the use of Microsoft Word for Windows, Excel, PowerPoint, Outlook and Internet
  • Understanding of personcentred support
  • Excellent communication skills
  • Able to use own initiative and work without direct supervision
  • Good observational skills
  • Practical problemsolving skills

  • Provide telephone advice and refer on or signpost to other sources of support.
  • Communicate and signpost to appropriate needs related information.
  • Guide people through the use of selfassessment resources.
  • To utilise verbal and non verbal communication skills to effectively communicate with patients, their families and carers. This may include patients/carers who may or may not have barriers to communication for example, language through cognitive or physical impairment.
  • Deliver Personal Care and Support Assessments and the development of a Personalised Care and Support Plan
  • Implement, monitor and review the Care Plan with the patient and carer, in line with standard operating procedures and protocols and modify as appropriate.
  • Evaluate outcomes of care delivery as appropriate.
  • Identify indicators of need or changes in need through telephone contact and respond appropriately.
  • Act as an advocate and facilitator to resolve issues that may be perceived as barriers to care.
