Pennine Care NHS Foundation Trust is excited to advertise a permanent role for a Band 5 Mental Health Practitioner with 1.5 years at least of experience in an appropriate mental health setting to work in the local Tameside Primary Care Network (PCN) along side a band 7 Senior mental health practitioner and a mental health wellbeing practitioner.

The introduction of these new roles supports the wider transformation of community mental health services for adults and older adults that seeks to bridge the divide between primary care and secondary mental health care, and physical health services.


The Mental Health Practitioner will be employed by Pennine Care but based within the Tameside Primary Care Network, within GP surgeries.

The Band 5 Mental Health Practitioner role promotes an improved access to evidence-based interventions for people with mental health needs.

The role will help people to focus on achievable goals, access local community resources and promote self-management.

The service will be for people over 18 years old with a specific focus on:


  • Those with mental health needs that exceed the access criteria for IAPT and do not meet the access criteria for secondary care
  • People with mental illness who are in recovery and relatively stable in secondary care mental health services who could be cared for within primary care
  • People with SMI who are eligible for an annual health check and Mental Health review and improved access to physical health interventions

The post holder will:


  • Provide mental health triage, basic assessments and clinical interventions based on NICE guidance within the neighbourhood
  • Support an access function for people, ensuring swift and seamless access to the most appropriate care pathway and onward access to living well mental health services
  • Hold regular mental health clinics based on the needs ofeach PCN orneighbourhood
  • Directly support GP's with the management of People presenting with Mental health problems and offer support to get the person to the right place for ongoing support and treatment
  • Support the Band 7 with people presenting in crisis, andproviding short term interventions, where possible, to manage the crisis, link in to mental health support networks and promote selfmanagement
  • Provide a link role between the Living well services and primary care services
  • Co facilitate groups with the Mental health well being practitioners
  • Clinically supervise Mental health well being practitioners
