Blackpool Teaching Hospitals NHS Foundation Trust are looking for substantive Speech and Language Therapists at Band 6 level on a full-time basis to join our Integrated Community Stroke and Neuro Rehab Service based at Ansdell Medical Centre.


It is an exciting time to join the team as we are currently working with the commissioning group to enhance our service to enable us to offer more to the people of Blackpool, Fylde and Wyre as well as developing a truly Integrated Community Stroke and Neuro Rehabilitation Service.


With all the new developments within the service, we are looking for someone who is motivated, keen and innovative to support the work that is currently underway and contribute in making the Integrated Community Stroke and Neuro Rehab Service truly great.

We will provide you with the IT equipment to support flexibilty to work from home, the office and in the community.

We are seeking to appoint an enthusiastic and motivated community Speech and Language Therapist for the ICSNRS based at Ansdell Medical Centre on a full-time basis with flexible working (agreeable on appointment).


The post holder will be an experienced speech and language therapist, ideally with knowledge of stroke and neurology rehabilitation but not essential.

As one of the community speech and language therapists, you will be providing a responsive and person-centred service, delivering the best client outcome through supporting and optimising peoples recovery at home, meeting their physical and mental health and social care needs and bridging the transition between acute hospital, primary and community care settings.


The post holder will provide holistic, individual assessments and rehabilitation in a variety of settings within the community, including patients own home, clinics, intermediate care and care homes for clients who have acquired a stroke or neurological diagnosis.


The post holder will enable our clients to achieve their goals to enable independence and improve their quality of life by working collaboratively across the different care groups.

The post holder will need to manage a clinical caseload of clients as a lone worker without direct supervision, however, support from the immediate and wider team is always available.

DUTIES AND RESPONSIBILITIES

CLINICAL

  • To demonstrate sound theoretical knowledge of the speech and language therapy process and skills necessary for the management of a range of swallowing and communication difficulties.
  • To be clinically responsible for the management of an identified adult caseload; to assess and review level of need and to organise this efficiently and effectively with regard to clinical priorities and time management principles.
  • To undertake comprehensive assessment of swallowing and communication needs, using an appropriate mix of standardised assessments, informal assessment and clinical observation skills.
  • To explain & discuss treatment options and develop an agreed individualised, outcomefocussed treatment plan with the stroke survivor and their family / carers using clinical knowledge and sound clinical reasoning skills.
  • To develop negotiated outcomefocussed therapy goals and assess progress towards these goals, making changes to planned interventions as appropriate.
  • To assess patient and partner/ carer understanding of treatment proposals and methods, gaining valid informed consent from appropriate person(s) taking into account factors such as age, mental capacity, social circumstances and individual responsibility.
