Description

We are seeking to recruit a Band 4 Occupational Therapy Assistant Practitioner to join our Occupational Therapy Service in Pembrokeshire.

This is an excellent opportunity for individuals with considerable experience working with people with complex healthcare or rehabilitation needs.


Our aim is for people to return to the community and to provide appropriate occupational therapy intervention to avoid readmission to hospital.


You will work within a supportive and busy Occupational Therapy Team, delivering assessment and rehabilitation to adults across the hospital, out reaching to facilitate earlier supportive patient discharge and maximising the function and skills of adults.

This role includes working in partnership with colleagues and alone.

You will have experience of using highly skilled handling techniques to facilitate patient movement, and/or rehabilitation.

You should be able to demonstrate clinical reasoning skills, to enable you to assess and treat patients within protocols without immediate direct supervision.


Part of the role will also be to support team operations, dealing appropriately with enquiries and undertaking administrative tasks relevant to the role.

Gather and collate information from a variety of sources to inform assessments, plans and interventions.


Independently undertake detailed observations, assessments and interventions on behalf of the occupational therapist, demonstrating a sound knowledge of the impact of the individual's conditions and circumstances on their function.

Plan and undertake treatment plans, interventions, and discharge with individuals following agreed guidance, protocols and pathways.

Review and modify the agreed plan, within service guidelines, informing the responsible occupational therapist.


Using knowledge of occupational therapy and the functional impact of health conditions adapt interventions and use initiative appropriately based on client's presentation.


Adhere to risk management plans when working with individuals and report any changes in risk status to the occupational therapist or care coordinator.


Work independently and alongside the Occupational Therapist and other members of the team, to deliver assessed therapeutic interventions across a variety of settings to include hospitals, person's home, and community & leisure facilities.

Deliver a variety of specialist interventions, requiring specific skills, including the use of equipment or therapeutic tools.


Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties.

Our 11,000 members of staff provide primary, community, in-hospital, mental health and learning disabilities services for around 384,000 people across a quarter of the landmass of Wales.

Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;

Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
48 general practices (five of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
Numerous locations providing mental health and learning disabilities services;

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.


Work as part of a designated team to deliver an effective and efficient occupational therapy service, as delegated by occupational therapists, to improve the health & wellbeing of people.
