Community Physical Rehabilitation Support Worker:
Band 3

Looking for an opportunity to support the improvement of peoples physical health and quality of life? What about a career that provides creative development opportunities within a team of supportive professional therapists?


Bedfordshire Community Health Services are looking for Rehab Support Workers to be part of the Transfer of Care Teams in North Bedfordshire.

We are a 7-day service supporting patient's home from hospital, please join us


Using a personalised and holistic approach, you will support individuals within their own environment, encouraging them to regain confidence and independence through meaningful tasks such as personal care, meal and bedtime preparation and exercise programs.

All visits are goal orientated via rehab plans. You will learn skills in moving and handling, risk assessing and identifying immediate solutions to reduce risks.

You will be supervised by a qualified member of staff, in order to provide you with support and upskilling.


There are promotion opportunities to work up to an Assistant Practitioner (band 4) which will be supported by senior management when opportunities arise.

All applicants need to have experience of working within a physical therapy service and ideally have community experience.

You will need to have or be willing to learn skills in moving and handling, risk assessing and identifying immediate solutions to reduce risk in order to provide for the needs and personal outcomes of the individual, their families and carers.


You will work closely alongside all members of the therapy team: Physiotherapists, Occupational Therapists, Assistant Practitioners and Rehabilitation Support Workers, as well as a wider multi-disciplinary workforce across Bedfordshire.

You will build close links with the professionals and services required to wrap around the needs of each individual to provide a holistic approach.


As a band 3, you will be supervised by a qualified member of staff within the team, in order to provide you with support, upskilling and to ensure your wellbeing.

You will support the qualified members of staff with student placements within the service.


For a band 3, there are promotion opportunities to work up to an Assistant Practitioner (band 4) and this will be supported by senior management when opportunities arise.


Rehabilitation Support Workers play a key role in shaping and developing the service provided and with this there will be opportunities to develop the role and service in a collaborative approach with Social Care colleagues and the acute settings.


All applicants will need to ideally have experience of working within a physical therapy service alongside Occupational Therapists and Physiotherapists or a clinical environment and have community experience.

You will also need to hold a full drivers license and have access to a vehicle.
