Job Summary


The Calderdale Intensive Home-Based Treatment Team for working age adults have a vacancy for a full time Band 6 senior practitioner.

Main duties of the Job


The post holder will be responsible for assessments, risk management, care-planning, and co-ordination of care delivery within a team approach.


You should be forward thinking, proactive and it is essential that you are willing to work flexibly as part of a 7-day week service, which includes shifts and working weekends and nights.

The role provides the opportunity to work across the health & social care spectrum as part of a team which has strong links to wider multi-disciplinary services including third sector providers.

The ability to form effective working relationships with fellow professionals, your team colleagues and service users is an essential requirement of the job and its a key requirement that you bring compassion to the role as the team engage with people in their daily lives.
