Description

An exciting opportunity has arisen for a Care -Coordinator to join our newly established Frailty Hospital at Home Team, which seeks to provide care for residents of nursing and residential homes across Buckinghamshire.


The post holder will work within Frailty Hospital at Home virtual ward providing administrative and where appropriate clinical support to facilitate the smooth running of the patient journey by ensuring that co-ordinated and streamlined administrative processes revolve around the patient and their individual needs.

This post will be approximately 5-10% clinically focused and 85-90 % administrative.
