The Community cardiology service includes provision of cardiac rehabilitation phase 2 and 3 services.

We have a fixed term opportunity for a band 4 Associate practitioner to join us to support cardiac rehabilitation. You should have exercise instructor level 4 qualification for cardiac rehab or equivalent.

You should have an understanding of the impact of cardiac disease on the physical and emotional wellbeing and be able to provide advice and support in a compassionate way to support our patients to progress through their cardiac rehab journey and leave able to self manage their conditions.


The community cardiology service also provides heart failure, one stop cardiac clinic, 24 hour tape, GP with Specialist interest and virtual ward services so you will have plenty of support from our specialist nurses and GPs.

You would be working along side our cardiac rehab nurses as well as other level 4 qualified associate practitioners.


As this is a fixed term post we are willing to consider those who do not currently hold the level 4 qualification as long as they can demonstrate knowledge of cardiac disease impact, lifestyle advice to support cardiac patients, and how to set exercise prescriptions within this patient population.


Cardiac rehabilitation is provided across North East Essex and a car driver with access to a car with business use insurance is essential.


  • To adhere to the policy and procedures of the community cardiology service
  • This will include delivering exercise programmes and health education according to patient needs.
  • Be able to provide initial medical attention if a patient becomes unstable and escalate to appropriate staff member quickly.
  • To measure and record manual blood pressure, pulse rate/rhythm, oxygen saturations, respiratory rate, blood glucose, height, weight and BMI in a home visit or clinic situation, identifying any deviation from acceptable normal for that patient and referring to a nurse any abnormalities.
  • Supervise Phase 3 Cardiac Rehabilitation exercise programmes and monitor patients during exercise in conjunction with the Cardiac Nurse Specialists
  • Adapt the exercise programmes appropriate to patient need
  • Deliver evidence based health education to cardiac patients as structured teaching within the cardiac rehabilitation programme as well as oneone with patients
  • Participate in pre
- exercise assessment as part of the risk stratification of patients prior to Phase 3 Cardiac Rehabilitation

  • Provide an alternative Phase 3 session for people unable to exercise fully, in the home environment if required
