Description

The objective of the PCN Digital and Transformation Lead is to support the PCN to deliver ongoing improvement to digital services to patients whilst ensuring high levels of patient satisfaction.

The post holder will lead and develop a strategic plan to build a robust digital infrastructure for Rural West PCN and to implement this, which; 1.

Is underpinned by a quality improvement methodology 2. Drives integrated working 3.

Drives efficiency and reduces duplication 4. Reduces variation between Practices 5.

Is underpinned with easy-to-understand data packs to support decision-making that includes but is not limited to demand, capacity, utilisation data, staff and patient experience 6.

Builds staff skills and capabilities 7.

Enhances patient access whilst protecting our workforce 8. Takes into consideration the digital maturity of the PCN 9. Takes into consideration the population's health needs and health inequalities within our community 10. Improves digital accessibility for our patients Leadership and Management 1.


To develop and maintain an in-depth understanding of the PCN digital infrastructure and business intelligence tools in conjunction with existing practice staff 2.

To ensure all change is ultimately focused on improving services for all patient groups 3.

To research digital tools for the PCN which is aligned to a framework of fundamental principles and seek to procure, in conjunction with Practices, in the most cost effective way, respecting that both Partnerships wish to remain separate 4.

To proactively project manage all digital projects and also evaluate the effectiveness of the intervention 5.

To ensure all initiatives are supported by a clinical champion and Practice management 6. To lead the development and implementation of digital transformation bids 7. To ensure all staff receive training on the digital tools available within the PCN 8. To ensure the induction of all new members of staff receive training and resources on tools available 9.

To ensure all initiatives are fully scoped and underpinned by a quality improvement methodology 10. To remain up to date with the national and local PCN guidance 11.

To be responsible for supporting the PCN to identify and make progress with digital transformational change, understanding and interpreting national and system-wide policy developments and initiatives to local circumstances and contexts Communication, relationship building and presentation skills 1.

To work in partnership with the PCN leadership and management team to understand current challenges to effective service delivery and identify solutions, including existing best practice within the PCN.


  • To develop and support delivery of transformational projects by engaging different stakeholders 3. To identify and develop relationships with practice staff, private providers, the local primary care team, neighbouring PCNs and other key partners to enable good working relationships and promote and share best practice 4.
