Primary Care Network (PCN) Coach, Manchester

What you'll be doing
As a PCN Coach within our Be Well service, you will work for the Ardwick and Longsight PCN.

Working with people who have multiple lifestyle issues that they wish to change - you will provide practical support on how to make these changes, help to deliver these changes with our community partners and get to know the neighbourhood you are working in.


What we need from you

  • You are committed to helping people make changes to improve their health and wellbeing.
  • You are passionate about the Manchester community and can work with people who face a range of barriers and social issues.
  • You speak Urdu and Punjabi.
  • You see the best in others, are inspirational and a good listener.
  • You can coach people (onetoone and in groups) to help them achieve their goals
  • You thrive on building effective working relationships with GP practices, partners and colleagues
  • You can manage a caseload of clients and keep our database records up to date
  • You will need an enhanced level DBS check for this role.

  • An annual salary of £26,845 NJC point 1

  • 25 days holiday, and 30 once you've been here five years
