KEY RESULT AREAS:

RECONNECT is a care after custody service that seeks to improve the continuity of care of people leaving prison or an immigration removal centre (IRC) with an identified health need, and reduce inequalities and address health-related drivers of offending behaviours.

This involves working with people before they leave to support their transition to community-based services, thereby safeguarding health gains made whilst in prison or an IRC.

Whilst not a clinical service, RECONNECT offers liaison, advocacy, signposting, and support to facilitate engagement with community-based health and support services.

RECONNECT aims to build on the existing Liaison & Diversion model to encompass the criminal justice pathway and beyond the prison gate.


The service will deliver care before and after custody to a cohort of prisoners released after remand or on completion of sentence and will support identified individuals on release from prisons within South Yorkshire who are resettling into the South Yorkshire region.

The service will provide screening, assessment, and practical support to individuals to engage with local services, during operational hours.

As a RECONNECT navigator, you will be an integral part of the wider South Yorkshire Liaison and Diversion service with a focus on assisting in the development of the RECONNECT project for prisons within South Yorkshire.

The postholder will, on behalf of South West Yorkshire Partnership NHS Foundation Trust, have authority to liaise with criminal justice agencies, healthcare, social care and services involved in the welfare of individuals in contact with the criminal justice system.


There is an emphasis in regard to liaison, the exchange of information, provision of specialist advice and guidance to enable access to treatment and securing support provision for these individuals.

The postholder will be required to work flexibly across South Yorkshire and will review mapped services across the region which have been utilised by the Liaison & Diversion Service to secure referral pathways for RECONNECT clients.

1 Care/service/function provision:

The postholder will:
be registered with the NMC. They will work within standard operating procedures / pathways to manage their delegated workload independently.


They will support the assessment of patients and implementation of programmes of care ensuring that they practice within their sphere of competence and knowledge.

The post holder will exercise professional accountability in those clinical areas in which they are competent. be responsible for their own caseload within their speciality area as an autonomous practitioner.

In doing so, they will assess, plan, deliver and evaluate patient centred programmes of care to a specialist level ensuring that they practice within their sphere of competence and knowledge.


The postholder will work to professional and regulatory body codes, standards and guidance at all times ensuring that their practice is grounded in evidence based theoretical and practical knowledge.

work within agreed framework to support assessment and review, plan, implement and evaluate clinical activities. recognise their own limitations ,acknowledging when to seek help and support. utilise evidence to inform decisions and solve problems associated with clinical care.

support and deliver care to a specific, delegated cohort of patients. support and assist other staff members to deliver care as delegated and directed. be able to make judgements involving facts or reviewing situations which may require analysis e.g. alter programmes of activities, updating care plans or making clinical decisions related to specific pathway of care.


identify risk or adverse or abnormal changes in condition and is responsible for taking actions and advising the lead clinician.

provide and receive routine or sometimes complex information using persuasion, reassurance, tact and empathy and addressing barriers to understanding in order to engage the individual in their care and gain valid consent for care and adjust care accordingly within their scope of practice.

maintain and complete documentation as per local protocol. adhere in all respects to the Nursing Associate Proficiency Standards.


approach each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of their role.

engage and actively involve the individual and, if appropriate, their family/carer, in their programme of treatment and/or care and its review by using person-centred techniques to promote a culture of self-care, involvement and empowerment.

manage their own workload and that of others and understanding the boundaries of their practice. Be responsible and accountable for clinical administration tasks and management of resources within the clinical area e.g.
