- Full Job Description: Care Coordinator_
The roles of our care coordinator will be to ensure all customer visits are allocated, to recruit suitable Care and Support Workers and to maintain appropriate office procedures.
Principal responsibilities of Care Coordinator:
- Answering incoming telephone enquiries.
- Processing new customer referrals.
- Experience with rostering
- Updating clients care plans / risk assessments as required
- Preparing and carrying out spot checks and wellbeing checks with completion of appropriate documentation
- Permanent allocation of Care & Support Workers to all customer visits and ensuring all visits always covered using the Birdie Care Platform
- Administration of all Individual Care & Support Agreements.
- Maintaining computer system up to date at all times.
- Ensuring all customer and Care & Support Worker documentation and electronic records are kept up to date at all times.
- Ensuring all other records are kept up to date.
- To work closely with the Senior Care Co-Ordinator
- To ensure compliance with all office systems and procedures.
- Carrying out direct care as required to ensure all care calls are covered
- Taking the on call phone on a Rota system
- Working hours are predominantly Monday to Friday, weekends may be required to meet the needs of the Care Calls with Clients
Salary:
From £26,000.00 per year
