Description
To monitor home visit requests and make initial assessments on the most appropriate person to visit, allocating and facilitating this process

Carry out home visits where clinically indicated for:

  • Acute (same day) requests
  • Nonacute requests to provide care of vulnerable patients such as vaccine administration, postdischarge review for patients returning home from hospital and to provide followup visits where necessary
Carry out on-site consultations focussing on minor illness/ injuries

Support multi-professional team (with additional training as required) with phlebotomy, wound care, ECGs, IM & SC injections, urine testing

Use available clinical systems, both static (on-site) and mobile to enter information into patients notes

Refer to GPs for prescribing needs and advice when required (unless independent prescriber), utilising PGD approval where necessary

Maintain close working relationships with GP mentor, multi-professional team members, care coordinator, community teams (including nurses, social care, other teams)

Pro-actively identifying patients who may benefit from case management or additional input and signpost/refer accordingly

Participate in care planning where appropriate, liaising with the care coordinator and the patients usual GP

Ensure good use of resources, including time, prescribing and other consumables

Play an active role in personal and team development were possible, participating in meetings and training when appropriate

Maintain and update knowledge and skills in accordance with personal and service requirements and participate in regular clinical supervision

Demonstrate own professional indemnity and continued protection throughout the life of the project.


Qualifications

Essential

  • Minor Illness Diploma or equivalent qualification/training
  • Basic Life Support
  • Experience of consulting patients both in the clinic and at home
  • Ability to drive and have access to a car

Job Types:
Full-time, Part-time, Permanent

Part-time hours: 37.5 per week


Salary:
£19.95-£21.30 per hour
