    Role Description
    This is a full-time on-site role for a Support Worker located in Wrexham.

    The Support Worker will be responsible for assisting individuals with their daily activities, implementing person-centred plans, and providing emotional support.

    Tasks include case management, coordinating with other services, and ensuring the health and wellbeing of individuals within the community. Daily activities may also involve attending meetings, documenting progress, and participating in training sessions.

    Qualifications
    Strong Interpersonal and Communication skills
    Experience in Community Support or Community Health
    Proficiency in Case Management
    Ability to work collaboratively with a team and other agencies
    Commitment to providing high-quality support services
    Relevant qualifications in social care or a related field
    Previous experience in a similar role is an advantage
