PCN Care Coordinator
This is an exciting opportunity to join the ever expanding and developing roles within Primary Care Networks (PCN).

PCNs are a cluster of GP practices working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices.


As a PCN Care Coordinator you will become an important member of a PCN multidisciplinary team, working in partnership with paramedics and the social prescribing link workers; taking referrals from all member practices, to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination of care and support across health, social care and other VCS services.


This is achieved by bringing together all the information about a person's identified care and support needs and exploring options from a multi-disciplinary approach to meet these within a single personalised care and support plan, based on what matters to the person.


KEY RESPONSIBILITIES

  • Work with people, their families and carers to improve their understanding of their newly diagnosed long term condition/s. Assist people to access information and selfmanagement education courses, peer support or interventions that support them in managing their health and wellbeing.
  • Liaise and work closely with the PCN paramedics following visits to facilitate any required coordination
  • Work with practices to identify their most vulnerable patients with complex care planning and or multiagency intervention requirements.

ABOUT YOU


You will have responsibility for a quota of patients, triaging referrals, providing information and support, actively connecting people to other PCN roles, specialist services or community services to promote independent healthy living.


Your role involves working with people who will be experiencing complex issues, so you will need good listening and communication skills and be able to build effective partnerships within GP practices and with a wide range of organisations and community groups.


You'll need excellent IT and organisational skills, be adept at seeking out the information for relevant services and have a tenacity to help people achieve sustainable outcomes and improvements in their health and wellbeing.

(37.5 hours per week)


Salary:
£28,840 per year


Schedule:
Monday to Friday - 9.00am - 5.30pm
