1. Purpose of post_


The post-holder will provide face-to-face Befriending, Information, Advice and Guidance support service to patient's band carers based in Ysbyty Cwm Cynon who are experiencing cognitive impairments including dementia who may have a wide range of social,emotional, or practical needs.

The role will focus on improving mental health and physical wellbeing offering a safety net and empower them to engage within their community creating a network of support.

2. Key tasks/duties_

As post-holder, you will be responsible to the Director for the following:

  • To be involved in recruitment and interviewing volunteers as required and assess the suitability of their skills and availability to provide a high-quality Community Liaison Service.
  • The Community Liaison Coordinator, support and work with individuals who may have a cognitive impairment including dementia and provide information and advice to their families as required.
  • Provide and deliver high quality, unbiased and impartial face to face information, advice, and guidance, designed to inspire the individual into making better informed career choices.
  • Deliver information, advice and guidance on all matters relating to the needs of individuals on a 1:1 or group settings, using appropriate resources and tools.
  • The Community Liaison Coordinator will ensure a range of set quantitative and qualitative targets are met using a case load management approach.
  • Provide befriending support to enable individuals to access local information and services to enable individuals to make informed decisions.
  • Continuously track carers journeys to achieve all relevant contractual outcomes using The Warwick-Edinburgh Mental Wellbeing Scales.
  • Provide referral support to individuals to enable them to implement their decisions, including liaising and attending initial meetings with the individual with other service providers and adopting an advocacy role when required.
  • Take a proactive and highly visible approach to developing working relationships with other professionals both internal and external, including provision of feedback, agreeing priorities, and reviewing arrangements.
  • Develop and maintain a full understanding of government initiatives to help individuals to make informed choices on the range of services and opportunities available to them.
  • The Community Liaison Coordinator will be responsible for safeguarding and promoting the welfare of all individuals accessing the service.
  • Maintaining and regularly updating the database system within agreed timescales to ensure individuals records are maintained and contractual requirements fulfilled.
  • Undertake relevant CPD as appropriate to maintain your professionalism.
  • Develop and maintain personal and professional effectiveness by participation in supervisions and performance review processes/observation of professional practice and by attending relevant working groups, training, and support activities.
  • Adhere to company policies and procedures.
  • Undertake such other operational duties as may be required from time to time as directed by your line manager.

  • To ensure volunteers are appropriately supported, inducted, and mentored.

Requirements:


  • Educated to GCSE standard or equivalent and the ability to undertake further learning
  • Organising and implementing own work including writing and submitting reports as requested
  • Experience of using Word, Excel, Outlook software
  • Experience of multiagency working (Social Services, Health Boards, Third Sector and Families)
