Reablement Coordinator
To effectively lead a group of reablement support workers to deliver a high-quality service with the aim of promoting and sustaining people's independence living in the community.
To liaise with the Rapid/Community Reablement Manager and all members of the team to ensure that service delivery throughout the reablement pathway is well coordinated and complies with organisational policies, procedures and Care Quality Commission regulated guidance.

Reablement Coordinator
We are looking to recruit in the vale royal area and surrounding rural area of the borough
You will be responsible for supporting reablement workers on a 1-1 basis mentoring / supervising staff in this role within the reablement team to deliver a high quality service with the aim of promoting and sustaining people's independence living in the community.
Completing a service user journeys well which coordinated and complies with organisational policies, procedures and Care Quality Commission regulated guidance. To ensure that the reablement service achieves the best quality outcomes for service users, their carers and families.
