Description

To be responsible for the SNOMED coding of relevant information into patients' electronic medical records on the EMIS Web clinical system.

Code relevant QOF / IIF / Enhanced Service data to support effective patient recall and to optimise income by ensuring the completeness of medical records.

Furthermore, to summarise the records, both paper and electronic, of all new patients.


  • Open all incoming post and distribute to all concerned as per the protocol and the preparation of incoming clinical post for scanning.
  • Prepare, scan and code all referral forms and cards. Scan all confidential letters/forms/reports etc.
  • Scan clinical correspondence and workflow to the appropriate clinician.
  • Handle the Incoming Mail inbox on Docman. Following the protocol workflow appropriate letters to the Doctors or directly to Data Input for coding etc.
  • Ensure Out-of-Hours' call sheets are processed every morning and printed for the Doctors to review.
  • Manage the Data Input Docman inbox. Input relevant information from all incoming mail, including hospital letters and discharge notes. Be aware of QOF, IIF, Enhanced Services and Safeguarding codes, ensuring information is captured to highlight to clinicians, add diary dates and code repeat tests e.g. bloods.
  • Workflow letters back to clinicians where it is not clear that all actions have been completed.
  • Summarise the notes of any patients requesting On Line Access to their medical records, before access is granted.
  • Update existing patient's computer record in a systematic process, adjusting diary dates if outstanding and linking drugs.
  • Manage the GP2GP Inbox in EMIS Workflow Manager.
  • Summarise the medical records of all new patients registering with the practice. File letters and hand-written notes in date order. Enter all relevant details into the correct areas on the patient's computer record. Be aware of QOF, IIF and Enhanced Services and ensure a patient's medication matches their problems.
  • Attend to tasks sent to the Team.
  • Assist with the preparation of the patient records to be sent for scanning and/or offsite summarising.
