An opportunity has arisen for an experienced Paramedic(s) to join Phoenix Health Primary Care Network.
The role will involve working across our 3 member practices:
Cross Key Practice, Haddenham Medical Centre and Unity Health to provide care to our elderly population living in the community and residents of care homes, working with our Frailty & Care Homes Team.
As part of our multi-disciplinary team and in conjunction with practice teams the paramedic will assess and treat minor illness and provide acute reactive care by way of a daily home visiting service.
Scope and purpose of the role
- To manage a clinical caseload and respond to presenting patient's needs in a general practice setting, on domiciliary visits and in care homes.
- To deliver a high standard of patient care using advanced, autonomous clinical skills with indepth theoretical knowledge and evidencebased practice.
- To provide clinical leadership within the multidisciplinary team, working with other members of the team to develop the services provided by Phoenix Health PCN.
Primary Duties and Areas of Responsibility
- Provide acute, reactive care as part of a multidisciplinary team, working in general practice and care homes or doing home visits. This shall include assessment, diagnosis, planning, implementation and evaluation of treatment from a physiological, psychological and social perspective.
- Communicate proactively and effectively with all colleagues across the multidisciplinary team, attending and contributing to meetings as required and accepting referrals and referring to specialist services where appropriate.
- As part of the multidisciplinary team, helping to develop Personalised Care and Support Plans (PCSPs).
- Consider aspects of care including; hydration and nutrition support, oral health, mobility, strength and balance, falls prevention, continence, skin Care, mental health including dementia.
- Provide support with vaccination programmes.
- Support the delivery of anticipatory care and end of life care plans.
- Undertake home visits in accordance with the relevant protocols.
- Refer patients to an alternative care setting or treat and discharge as appropriate. Signpost and where appropriate refer patients to other community or voluntary services, PCN services and to selfcare.
- Support the achievement of PCN and practice quality and contractual targets markers including:
- Input into COPD, Asthma, Diabetic and Dementia annual reviews and other QOF requirements, performing health checks and reviews.
- Input into PCN IIF and Care Homes SNS targets.
- Perform investigatory procedures as needed by patients and those requested by GPs and the wider PCN, including collection of pathological specimens including intravenous blood samples, swabs, etc.
- Undertake clinical audits.
