Our goal is to provide an accessible, friendly, evidence based, and compassionate service and you will have a vital role to play in joining our newly created team of Care Coordinators.

Join us on our new venture to provide dedicated, person-centred care coordination to some of our more vulnerable patients.


This is an exciting opportunity for a highly motivated and passionate person who wants to support patients by coordinating their health and social care service needs.

Each of our Care Coordinators will be assigned to a specific GP team and will be involved in supporting the clinical team to identify and provide proactive, person-centred care planning.

The whole team will be new; so you'll learn, grow and reach new exciting goals together


The Care Coordination team will provide a key point of contact, for groups of patients, each within their aligned GP Team (for example palliative, mental health patients, those with long term conditions), allowing the Partnership to provide excellent continuity of care.

They will be helping patients to navigate health and social care, and supporting them to understand and manage their conditions, ensuring their changing needs are addressed.

This could include a wide range of services, such as hospital care, community care, social care, housing, and voluntary sector services.


You will deliver a professional, efficient, and proactive co-ordination service, with the support of newly appointed Care Coordinator Team Lead, who will coach and support you to meet your objectives and KPIs.

A key focus of this role will be supporting the mental health and wellbeing of our patients by connecting them with the right services, therefore any experience in mental health or a passion to learn more would be an advantage in this position.


We're happy to support you in your ongoing development; so long as you have prior experience in health and social care, can demonstrate your ability to support patients effectively and empathically with a range of physical and mental health conditions, and are confident and proactive.

Must be available Monday

  • Friday until 5pm each day with 1 day per week until 18:30

£22,485.00-£25,610.00 per year
